When a product recall or safety notice occurs, we are committed to providing timely, accurate information and clear instructions for our customers. This page contains current recall notices for affected products produced by AvKARE. Each recall notice includes detailed information about the product, reason for the recall, and return instructions.

If you have purchased an affected product, please select the appropriate recall notice below. Complete the “Quantity to Return” and Customer Information sections, even if the quantity is zero. Electronically submit your response by selecting Electronic Recall Form. Alternatively, you may complete and fax the downloaded form to 931-292-6229 or email to recalls@avkare.com.

Please examine your inventory to determine if you have any of the lot’s listed. If so, immediately discontinue use and remove it from active inventory.

IF YOU HAVE PREVIOUSLY SENT A REQUEST FOR A RETURN AUTHORIZATION, PLEASE ALLOW EXTRA TIME FOR THE RESPONSE.

Once you submit your response, we will send you a Return Authorization Form (RA). Please return any recalled product that was purchased from AvKARE to the following address. Also, include a copy of your completed recall form in your return package.

We will issue full credit including shipping cost for any returns. If you have any question on this matter, please contact us. We regret any inconvenience that this may cause.
